Hardcore Free For All?

I don't understand why this mode is always left out of call of duty games. It's happened the last 3, but BO2 added it in about a month later. Is there a specific reason? It used to rank up in the top 5-6 in popularity out of all of the game modes.

There's a very niche audience for this mode, but I'd expect Sledgehammer to add it in if enough people request it.
